Services Golden Grace Offers

Our medical and Nursing staff assess each member to determine their needs for personal care and assistance in performing activities of daily living such as bathing, dressing, grooming, toileting, and eating and include their needs in the individualized plan of care and supervise the provision of these services.

The nursing service shall be as:
Provide skilled nursing care
Provide Health education
Monitor, administer and record prescribed medication
Provide liaison between the member, physician and caregiver
Observe and monitor health issues such as Glucose level, blood pressure , etc.

Most people with Dementia still have energy and desire to continue with activities that he or she loves. At Golden Grace Center we modify activities to meet our members abilities, in addition to enhancing quality of life. Well designed activity can reduce difficult behavior such as confusion, agitation, and wandering and it will improve mood.

Our activity program provides planned schedule of social and other purposeful independent and group activities. Our members are encouraged to participate in activities suited to their individual needs, their abilities and level of functioning.

Our primary Objective of our activities program is to encourage the participant toward self restoration to self care and resumption of normal activities or for those who cannot realistically resume normal activities to prevent further mental or physical decline.

Our well designed activities are based on:
* Members skills and abilities. We pay special attention to what the person enjoys and not what the person could achieve.
* Consider physical and mental abilities.
* Ask for favorites.
* Not to correct or criticize but encourage and motivate.

At Golden Grace Day Activity and Health Services our nutritional services are:

* Safe and Healthy
* Appetizing
* Hot nutritional lunch, Continental Breakfast and snacks are served.
* Therapeutic diets prescribed by member’s physician are provided.
* Nutrition counseling is available to our members and caregivers by Registered Dietitian.
